To request use of an AMAFCA facility for a special event, please complete the Special Event Use Request Form below and attach a current Certificate of Insurance (COI) meeting AMAFCA’s insurance requirements.
Submitting a request does not authorize use of the facility. All requests are subject to AMAFCA review and approval, and a Special Event Use License must be fully executed before the event may take place.
